Pricing

This clinic offers an inclusive participant-focused primary care model devoted exclusively to the prevention and treatment of Alzheimer’s disease and dementia. They focus on the prevention of and treatment for participants:

1. At risk for or concerned about Cognitive Decline
2. Subjective Mild Cognitive Impairment
3. Mild Cognitive Impairment
4. Diagnosed Alzheimer’s disease

This clinic includes support from the following providers, either in-clinic or by dedicated referral:

1. Medical Doctor
2. ReCODE 2.0 Certified Family Nurse Practitioner
3. Health Coach
4. Nutritionist
5. Exercise Coach

Treatment therapies such as Hyperbaric oxygen therapy, Exercise With Oxygen Therapy (EWOT), Neurofeedback, Heartmath, and Cryotherapy are referred out, as recommended.

For participants outside of Florida, this clinic’s practitioners require an in-person visit to establish the relationship and then at least one time in-person each year to follow. Follow-ups via telemedicine as appropriate.



NEW PARTICIPANT PROCESS

• Clinic will explain the New Participant Process and costs, their comprehensive inclusive program pricing for that includes: baseline lab testing, ReCODE Reports or PreCODE Reports, all clinic visits and virtual encounters, basic nutraceuticals and additional services (such as: coaching, ketone meters, supplements, nutrition counseling, etc.)
• Participant completes intake packet and signs the participant agreement (in-clinic or at home)
• Determine initial participant status, risks, prior diagnosis, if any
• Participant completes the Cq Assessment (outside the Apollo Health paywall): https://www.apollohealthco.com... (if no prior diagnosis)
• Clinic will contact participant once results are received from the Cq Assessment to schedule blood draw for ReCODE or PreCODE labs as indicated* (in-home, at clinic, or at a LabCorp location) and the REPORT REVIEW VISIT.
• Clinic guides the participant to appropriate complete the next cognitive assessments for ReCODE (MoCA test or SLUMS exam, AQ-21, and CNS Vital Signs [taken on the Apollo Health website]) or PreCODE (MoCA test or SLUMS exam, and Cq Assessment [taken again, inside the paywall, on the Apollo Health website])
• See REPORT REVIEW VISIT for next steps.

REPORT REVIEW VISIT (90 minutes)

• Once blood and cognitive test results come back, participant will come in for their first visit with the ReCODE practitioner to:
• Review the personalized ReCODE Report or PreCODE Report
• Review SLUMS or MoCA scores and CNS Vital Signs or Cq Assessment results
• Review medical history
• Answer questions to further understand history
• Discuss additional necessary screenings such as depression/anxiety/stress, sleep, exercise, nutrition, inflammation, toxicity, advanced genetic testing, based on the participant’s history and the results in the ReCODE Report or PreCODE Report
• Develop Participant’s Care Plan including: nutrition and exercise plans, supplement recommendations*, monitoring glucose and ketones, getting started with BrainHQ brain-training, and completing monthly CNS Vital Signs or Cq Assessments (to track your progress)
• Answer any questions you have about the protocol as you are getting started

*Cost of basic supplements (as recommended by the ReCODE practitioner and ReCODE Report or PreCODE Report) included only if the participant moves forward with the YEAR ONE PROGRAM.


NEW PARTICIPANT PROCESS AND REPORT REVIEW VISIT (described above): $1800* (all participants)
*The cost will be applied to the YEAR ONE PROGRAM price when the participant moves forward with OPTION 1 (described in the YEAR ONE PROGRAM section below).
This clinic will:

• Contact participant once results are received from this Cq Assessment to schedule blood draw* (at home, in clinic, or at a LabCorp location)
• Once blood and cognitive test results come back, participant schedules for first appointment (REPORT REVIEW VISIT) with a ReCODE practitioner to review the ReCODE Report or PreCODE Report, discuss the results and proposed care plan.

OPTION 1

MOVING FORWARD WITH THE YEAR ONE PROGRAM
This year-long program is split into 4 phases (three months each) and includes:

• NEW PARTICIPANT PROCESS (mentioned above)
• REPORT REVIEW VISIT (mentioned above)
• Ketone meter
• All Follow-up Visits (in-clinic and telemedicine) are included in program price and will be provided in accordance with the participant’s condition, progress, desires, and clinical team judgement
• One-year Apollo Health subscription to ReCODE or PreCODE
• One set of baseline labs for ReCODE or PreCODE
• Baseline ReCODE Report or PreCODE Report
• Updates to the ReCODE Report or PreCODE Report to add additional data (for a deeper dive) or updated data (to track your progress over time)
• Additional diagnostic labs and imaging (available at an additional cost, as recommended by the ReCODE practitioner).
• Updated lab testing* over time to track your results in follow-up ReCODE Reports or PreCODE Reports as guided by the ReCODE practitioner
• Basic supplements as recommended by the ReCODE practitioner and ReCODE Report or PreCODE Report
• Delivery and modification of care plan (coaching, nutrition, sleep monitoring, glucose ketone monitoring, food journaling, exercise, stress reduction practice, BrainHQ, and more) based on current results

*Repeat labs tests may be recommended over time to track your progress. The cost of these lab tests are not included in the program price.

YEAR ONE PROGRAM: $15,000

• PHASE 1: $5,000
• PHASE 2: $4,000
• PHASE 3: $3,000

PHASE 1 (months 1-3): DIAGNOSTIC AND INITIAL ASSESSMENT

• During the first three months in addition to the NEW PARTICIPANT PROCESS AND REPORT REVIEW VISIT, participants have access to:
• Unlimited scheduled Visits* with a ReCODE practitioner
• Unlimited scheduled Coaching Visits* (in-person or via telemedicine based on participant’s desires)
• Support provided for BrainHQ brain-training, and implementing nutrition and exercise recommendations
• Practitioner review of any follow-up or additional testing results and cognitive assessments, primary care, management of referral to specialist
• Personalized supplement recommendations
• Based on the findings of the baseline ReCODE Report or PreCODE Report and any deeper dive testing**, the clinic will provide a Care Plan including unlimited follow-up Visits with a ReCODE practitioner and unlimited scheduled Coaching Visits, to best support the participant’s individual needs and desires as they implement the protocol.

*Some Visits may be shorter or longer than others. Visit duration and frequency are determined by participant’s condition.
**Additional testing including but not limited to: ARC-R, Metal Toxicity, Mold Toxicity, Food Sensitivity, Imaging, Inflammation, and Sleep Studies at additional cost.


PERSONALIZED CARE PLAN
Clinic will develop a personalized Care Plan for each participant, consistent with the Participant’s condition as indicated, status, and progress, which may include:

• Unlimited scheduled Visits* with a ReCODE practitioner
• Unlimited scheduled Visits* for Coaching Support
• Initial and Periodic Cognitive Assessments
• Initial and Periodic** diagnostic testing
• Detoxification/remediation**
• Exercise, KAATSU*, and Exercise With Oxygen Therapy (EWOT)**
• Nutrition recommendations and nutrition counseling support
• Basic supplements as recommended by the ReCODE practitioner and ReCODE Report or PreCODE Report
• Health monitoring recommendations, including as indicated: Glucose monitoring (and journaling) and Ketone monitoring (and add to journal: BHB or ACES, mood, energy, and cognition)
• Sleep studies** or Sleep monitoring** (blood oxygen, etc.…)
• Mold Remediation**
• Hyperbaric oxygen therapy**
• Photo Biomodulation** and Neurofeedback**

*Some Visits may be shorter or longer than others. Visit duration and frequency are determined by participant’s condition.
**Cost of (some) lab testing, treatments, KAATSU, Exercise with Oxygen Therapy (EWOT), mold remediation, Hyperbaric oxygen therapy, and Photo Biomodulation with Neurofeedback are not included in program price.


This clinic will provide comprehensive primary care and management of referral to specialists. After three months (PHASE 1), next steps will be determined for the participant (PHASE 2).


PHASE 2 (months 3-6): PROTOCOL IMPLEMENTATION AND STATUS

• During the second phase and following, participant has access to:
• Continued unlimited scheduled Visits* with a ReCODE practitioner
• Continued unlimited scheduled Coaching Visits* (in-person or via telemedicine based on participant’s desires)
• Support provided for BrainHQ brain-training, and implementing nutrition and exercise recommendations
• Practitioner review of any follow-up or additional testing results** and cognitive assessments, primary care, management of referrals to specialists
• Ongoing personalized supplement recommendations

* Some Visits may be shorter or longer than others. Visit duration and frequency are determined by participant’s condition.
** Repeat labs tests may be recommended over time to track your progress. The cost of repeat and additional lab tests are not included in the program price.


Near the end of PHASE 2, next steps will be determined for the participant.
These will include:

• Discussion and completion of repeat or additional testing* to include in your updated ReCODE Report or PreCODE Report (to track your progress)
• Review updated ReCODE Report or PreCODE Report with ReCODE practitioner
• Continuation or adjustments to care plan (see PHASE 1 description of components of PERSONALIZED CARE PLAN) based on new results
• Continued coaching support
• Schedule next follow-up Visits or calls to continue care plan

* Repeat labs tests may be recommended over time to track your progress. The cost of repeat and additional lab tests are not included in the program price.


PHASE 3 (months 7-9): PROTOCOL IMPLEMENTATION AND STATUS
Implementation of next steps and adjusted care plan (see PHASE 1 description of components of Care Plan) During the third phase and following, participant has access to:

• Continued unlimited scheduled Visits* with a ReCODE practitioner
• Continued unlimited scheduled Coaching Visits* (in-person or via telemedicine based on participant’s desires)
• Continuation or adjustments to care plan (see PHASE 1 description of components of PERSONALIZED CARE PLAN) based on new results
• Support provided for BrainHQ brain-training, and implementing nutrition and exercise recommendations
• Practitioner review of any follow-up or additional or updated testing results and cognitive assessments, primary care, management of referral to specialist
• Ongoing personalized supplement recommendations

* Some Visits may be shorter or longer than others. Visit duration and frequency are determined by participant’s condition.


PHASE 4 (months 10-12): PROTOCOL IMPLEMENTATION AND STATUS
The fourth phase includes:

• Continued unlimited scheduled Visits* with a ReCODE practitioner
• Continued unlimited scheduled Coaching Visits* (in-person or via telemedicine based on participant’s desires)
• Discussion and completion of repeat or additional testing** to include in your updated ReCODE Report or PreCODE Report (to track your progress)
• Review updated ReCODE Report or PreCODE Report with ReCODE practitioner
• Continuation or adjustments to care plan (see PHASE 1 description for PERSONALIZED CARE PLAN)
• Discussion of continued care moving into YEAR TWO PROGRAM (see pricing schedule below)
• Ongoing personalized supplement recommendations

CONTINUED CARE

YEAR TWO PROGRAM: $12,000
Continue the progress made in the YEAR ONE PROGRAM:

• PHASE 1: $3,000
• PHASE 2: $3,000
• PHASE 3: $3,000
• PHASE 4: $3,000

OPTION 2

MOVING FORWARD WITH COACHING ONLY (3 months): $1,200*
After completing the NEW PARTICIPANT PROCESS, if participant desires prevention, and would only like to continue working with a Health Coach after the PreCODE REPORT REVIEW VISIT. This option includes:

• Unlimited scheduled Coaching Visits (in-person or via telemedicine based on participant’s desires)
• May include updates to PreCODE Reports over time, as indicated by participant condition and provider judgment as well as availability of new testing**, as appropriate.

*Monthly payments of $400 for three months.
** Supplement and lab costs are not included.
